What you need to know
When you schedule a complimentary consultation with us, you will receive an automated information pack that includes:
- Welcome Email: Contains details about our service fees, payment instructions and Locations for Supervised Family Time Visits document that helps parents by detailing available locations for supervised family visits and ensuring they select safe, appropriate venues for their child’s well-being, with clear guidelines on approved and high-risk settings.
- Online Forms: Includes the Service Agreement, Referral Form and Financial Agreement.
To minimise our carbon footprint, all agreements are sent electronically. Once completed, these documents automatically generate a receipt and copies of the signed agreements for all parties’ future reference. Both the residential and supervised parents must sign these documents separately; upon completion, they are combined into one binding agreement between Cherished Connections and the parents. To preview these documents, please see below, where you can view the forms in a read-only format
Children’s Contact Service Agreement
The Children’s Contact Service Agreement is a formal contract between Cherished Connections and the parents with parental responsibility for the child(ren). It outlines the terms for services provided, including supervision and scheduling of visits. By signing, both the resident and supervised parents agree to follow the outlined procedures and responsibilities to ensure a safe and respectful environment for the child(ren). Any changes to the agreement will be communicated in writing, and compliance with the terms is necessary for continued service.
Referral & Financial Agreement
The Referral Form must be completed with agreed visit details, including timing, venues, and transport, and signed by both parents before submission to CC. Parents are responsible for finalizing arrangements and obtaining consent for changes. CC strives for consistent facilitators, with backups available if needed. Visits begin only after the form is signed and payment is cleared. Attendance should be limited and pre-approved. Provide complete transport and venue details, and adhere to the scheduled sessions to avoid service cancellation. Non-payment may result in legal action.
Service Fees & Payment Instructions
Our documents provide clear rates and payment instructions. The responsible party, as stated in the Referral Form, covers all fees. An intake fee applies, and supervision payments are due at least 2 days in advance. Accepted payment methods include bank deposit, transfer, debit/credit card, PayID, or EFT, with possible card surcharges. For weekend visits, payment is due by 5 PM Thursday. Additional charges for mileage, tolls, and venue costs may apply. Payments must be made to the specified CC bank account, and confirmation is required to prevent cancellation.
